All students will have to provide a Health History form and an Immunization Form with signatures from the child’s physician and the parents. Parents must indicate if the child has allergies and dietary restrictions.

Please do not send any medication to the school in the child’s backpack. All medication must be prescribed by a doctor. The parents must bring the medication to the school and complete the medication form.

If a child becomes ill, we will bring the child to a secluded comfortable area to make them feel as comfortable as possible until the parent arrives.

Our goal is to help maintain a healthy environment at the center. If your child has any of the following contagious symptoms, we cannot allow the child to come into the center.

Fever

Persistent coughs that are longer than a week.

Vomiting

Diarrhea Conjunctivitis (Pink Eye)

Sore throat

Rashes on skin

Ringworm

Any diseases, such as COVID-19, Influenza, RSV, Scarlet Fever, Hand, Foot, and Mouth Disease.
